Personal Rights — Based upon facility documents and interviews with multiple parties, Child C2 was given Child C1's bottle to drink. This posed a potential risk to the health and safety of the child.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above. Staff M.M. did not have a current Physician's Report on file. This poses a potential health, safety or personal rights risk to persons in care.
Infant Care Personal Services — Based upon Kindercare app and RP interview, C1 was not changed often enough (4 hours between changes) which may have caused a rash and is a potential health, safety and personal rights risk to children in care.
Reporting Requirements — On 10/23/2023, it was alleged that a staff had pushed a child and yelled at a child in care. Facility did not report the alleged incident to the department. This poses a potential health and safety risk to children in care.
Criminal Record Clearance All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as staff Maryam Mosa has been working at the facility since August of 2023 and her fingerprint is not associated to the facility and a fax confirmation receipt is not available for review. This poses a potential health and safety risk to children in care.
