Personal Rights — , Based on interviews conducted, 2 of 2 staff members disclosed that they witnessed the Director speak inappropriately to S1 & S2 in front of the children. This is a potential health and safety risk to children in care.
Staff-Infant Ratio — , Based on interviews conducted, 5 of 6 staff members stated the toddler room will have children wake up early causing the teacher to be out of ratio for about 10-30 minutes on a daily basis until another staff member is able to step in. This is a potential risk to the health and safety of children in care.
Responsibility for Providing Care and Supervision for Infants — , based on interviews conducted 4 of 4 staff members stated that Child #1 was left outside for up to 5 minutes during a transition. This is a potential health and safety risk to the children in care.
Staff-Infant Ratio — , During visit, parent informed LPA that infant room was out of ratio and teacher needed support. LPA inspected the infant room and and spoke with Staff #1 (S1) and counted the 9 children. This is a potential health and safety risk to children in care.
Limitations on Capacity and Ambulatory Status — , Based on interviews conducted 4 out of 4 staff members and confidential witnesses, it was disclosed that C1 was moved from the infant classroom into the 2's classroom prior to their 2nd birthday to meet ratio. This is a potential health and ssafety risk to children in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 1 out of 5 persons missing physical & TB test in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation, the licensee did not comply with the section cited above in multiple shelves observed with many ants on dry food packages which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care General Sanitation — Based on observation, record review and interviews, the licensee did not ensure that the sanitation practices regarding flooring, rugs, walls and other areas that infants have access to were met, which poses a potential health and safety risk to persons in care.
