Staff immunization requirements — Based on record review, 1 out of 3 staff files reviewed were missing proof of immunizations, therefore the licensee did not comply with the regulation cited above which poses a potential health & safety risk to persons in care.

Personal Rights — Based on observation and interviews, the facility did not ensure children's personal rights, therefore the licensee did not comply the section cited above which posed a possible health and safety risk to persons in care.with

Personal Rights — Based on observation and interviews, S1 used physical intimidation towards children in care, therefore the licensee did not comply the section cited above which posed a possible health and safety risk to persons in care.with

Personal Rights — Based on observation, interviews and record review, S1 and S2 used verbal and physical agression towards children in care and did not ensure the children's personal rights , therefore the licensee did not comply with the section cited above which posed a possible health and safety risk to persons in care.

Responsibility for Providing Care and Supervision — The Licensee did not comply with the regulation cited above, where based on interview conducted, report received and LPA observations, a child was left without supervision for approximately (3) minutes, which poses/posed a potential health, safety, and personal rights risk to children in care.

Personal Rights — , Based on interviews conducted Staff #1 (S1) admitted to grabbing Child #1 (C1) by the ankle in the attempt to bring C1 down from the play structure, causing C1 to fall down. This is a potential health and safety risk to children in care.

Reporting Requirements — , Director sttaed that they did not notify CCL about incident that was reported to them. This is a potential health and safety risk to children in care.

Reporting Requirements — , Based on interviews conducted, Diretcor stated that 1 of the 2 childrens parents were notified of the incdent that was reported to them. This is a potential health and safety risk to children in care.

Personal Rights — , Based on interviews conducted, 2 of 2 staff members disclosed that they witnessed the Director speak inappropriately to S2 and slam the classroom door in front of the children. This is a potential health and safety risk to children in care.

Responsibility for Providing Care and Supervision — , Based on interviews conducted, 7 out of 7 staff members disclosed that Child #1 (C1) and Child #2 (C2) both got out of the Discovery Preschool (DP) classroom onto the side yard unsupervised. This is a potential health and safety risk to children in care.

Personal Rights — , based on interviews conducted 2 of 3 staff members disclosed that they witnessed S1 push Child #1 (C1) in the face area causing C1 to fall to the ground. This is a potential health and safety risk to the children in care.

Personal Rights — , based on interviews conducted 2 of 4 staff members disclosed that diapers had not been getting completed in a timely manner recently with S1.This is a potential health and safety risk to the children in care.

Teacher-Child Ratio — , Based on interviews conducted 3 of 4 staff members stated that they will be out of ratio in the mornings for up to 10 minutes and based on record review LPA observed on 5/28/25, 5/29/25 and 6/3/25 classroom to be out of ratio. This is a potential health and safety risk to children in care.

Personal Rights — , Based on 6 of 6 staff and confidential interviews that C1 had been having behavioral challenges since prior to 1/2025 when the old Director left the facility. However, based on record review and interviews facility did not contact the Inclusion Department for assistance until 3/2025 and then soon after the child dis-enrolled. This is a potential risk to the health & safety of children in care.

Limitations on Capacity and Ambulatory Status — , Based on 6 of 6 staff and confidential interviews that at least 7 preschool children were being moved from their prek classroom to the kinder/school-age classroom due to the children being older prek and to assist with behaviors, despite the children not being of age to advance to the school age program. This is a potential health & safety risk to the children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 persons missing updated Mandated Reporter certificates in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation, the licensee did not comply with the section cited above in multiple shelves observed with many ants on dry food packages which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — The licensee did not ensure epidemic outbreaks of fever, vomiting, and diarrhea were reported from January 3, 2024 to present day. This poses a potential health, safety, and personal rights risk to children in care.
