Teacher Qualifications and Duties — Based on interviews, observation and records review, Staff 1 does not have required courses to be a qualified teacher, and was left supervising children without a qualified teacher, which poses a potential risk to the health and safety of children in care.
(o)Playground equipment shall be securely anchored to the ground unless it is portable by design. (1) Equipment shall be maintained in a safe condition, free of sharp, loose or pointed parts. This Evidence was not met by: Based on observation, and interviews, the preschool play structure has a slide that is not securley attached to the play structure which poses a potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — Based on record review and interviews, the facility does not have current lead testing results which poses a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on interviews, record review, and evidenced gathered, staff were commingling children from different programs and utilizing off limit areas for the preschool children which poses a potential health, safety or personal rights risk to children in care.
Outdoor Activity Space — LPAs observed broken play structures, metal screw not properly mounted on the wooden board, deteriorating wooden beam, and deep hole on the ground under the rock climbing structure located in the preschool play areas. This poses a potential health, safety, and personal rights risks to children in care.
Limitations on Capacity and Ambulatory Status — Based on observation and record review, the licensee did not comply with the section cited above by allowing school aged children to commingle between preschool kids which poses a potential health, safety or personal rights risk to persons in care.
(1) On or before March 30, 2018,... a licensed child day care provider, administrator, or employee of a licensed child day care facility ... shall complete renewal mandated reporter training every two years ... Based on record review, the licensee did not comply with the above regulation. One teacher S1 didn't have have current mandated reporter training certificate on file, which poses an potential Health [and/or], Safety [and/or], Personal Rights risk to persons in care.
Admission Agreements — Based on infromation obtained Director sent out a notice of increase child monthly rates to parents on February 8, 2024 effective March 4, 2024. Director failed to give a required 30 day written notice. This poses a potential risk to the health and safety of children in care.
