Personal Rights — It was determined that a shelving unit was tipped over, forcing a child to fall out of the unit onto the floor which posed an immediate risk to the Health, Safety or Personal Rights to children in care.

Personal Rights — A child was confined to the bathroom as a form of discipline which posed an immediate risk to the Health, Safety or Personal Rights to children in care.

Personal Rights — During an incident when a child was forced out of a shelving unit, the shelving unit was tipped over by the staff member, causing toys in and on the shelf to fall on the child and other children in that area of the classroom which posed an immediate risk to the Health, Safety or Personal Rights to children in care.

Responsibility for Providing Care and Supervision — C1 was left in the classroom unsupervised on 0/23/25 which poses a immediate risk to the health and saftey of children in care.

Teacher-Child Ratio — According to staff, the facilicty has operated out of ratio on several occasions which poses a potential risk to the Health, Safety or Personal Rights to children in care.

Personal Rights — Staff yelled at children and spoke to children in a condescending manner which posed a potential risk to the Health, Safety or Personal Rights to children in care.

Reporting Requirements — The facility did not report to licensing an incident that occurred where a child was injured and received medical treatment.

Personnel Records — Based on record review, the licensee did not comply with the section cited above in 2 out of staff which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 1 child which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on record review, the licensee did not comply with the section cited above when Director did not contact licensing regarding an incident in a timely manner which poses an potential risk to the health, safety or personal rights of children in care.

Responsibility for Providing Care and Supervision — C2 was left in the classroom unsupervised which poses a potential risk to the health and safety of children in care

Teacher Qualifications and Duties — S8 was hired as a teacher. S8 does not met teacher qualifications. LPA observed S10 supervising children alone. S10 does not meet the required qualifications for a teacher which poses a potential risk to the health and safety of children in care.

Teacher-Child Ratio — an aide and teacher were supervising more than 15 children which is a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision — C1 was left in the classroom unsupervised on 9/17/24 which poses a potential risk to the health and saftey of children in care.

Drinking water lead testing — The children in care bring water bottles to the facility and the center uses a filtered water system. Based on interview, the facility has not had its drinking water tested for lead contamination levels by January 1, 2023 which poses a potential health, safety or personal rights risk to persons in care.
