School-Age Child Care Center Teacher Qualifications and Duties — Based on observation, S1 was observed supervising school age children alone without the required credentials. This poses/posed a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on observation and director admission, the facility did not comply with the section cited above. The center did not get their water tested for lead poisoning and is utilizing the faucets for drinking water. This poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. 1 out of 3 staff do not have their immunizations on file. There was no proof of measles (MMR), pertussis (Tdap) and/or flu shot on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
