Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility tour the LPA reviewed a fire drill log which shows the last documented fire drill on 2-27-2025.

During the staff file review the LPA was unable to review a valid CPR/1st Aid certificate for S1 or S2.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for C3.
Mandated reporter training — Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the interview with the Licensee she disclosed her or her Assistant do not have an updated Mandated Reporter certificate.

Child's Records — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the review of children’s file LPA was unable to review an LIC 9150 and LIC 627 for C3 and C5.
