Family home — preventive health training, including pediatric CPR and first aid — LPAs reviewed pertinent employee documents and observed that licensee's Pediatric First Aid and CPR certification expired on 3/13/2025.
Mandated reporter training — assistant P. Ravaei does not have a valid Mandated Reporter Training.
Immunizations — 2 out of 5 children's file did not have immunization record on file.
Staff immunization — influenza, pertussis and measles — assistant's file was incomplete, proof of immunization record was not available for review. (MMR, Dtap and Flu vaccine and/or waiver)
