Responsibility for Providing Care and Supervision for Infants — Based on observation, the licensee did not comply with the section cited above in 4 out of 4 children were left without visual supervision in the napping room, which poses an immediate health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Upon arriving to the facility, LPAs observed Staff #1 working at the facility prior to Licensee requesting a criminal record clearance which poses an immediate health, safety, and personal rights risk to children in care.
Responsibility for Providing Care and Supervision for Infants — Based on the review of the video and interviews, staff did not provide care and supervision necessary to prevent C1 from sustaining bruise on their right ear after being hit by another child in care, which poses an immediate Health or Safety, or Personal Rights risk to persons in care.

Food Service — Based on LPAs observation, the facility did not comply with the section cited above. During today's inspection LPA observed 3 opend expired food containers and 1 opened expired beverage in refrigerator, which poses/posed an potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 4 out of 4 children records were missing an authorized representatives signature and Director signature on Infant Needs and Service Plan,which poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation , the licensee did not comply with the section cited above in one unlabled opened medicine container in infants refrigerator with out documentaion from authorized representation, which poses/posed a potential health, safety or personal rights risk to persons in care.
