Drinking water lead testing — Director stated no lead testing was conducted for the facility .
Outdoor Activity Space — Based on observations , the licensee did not comply with the section cited above, during the tour with the Director LPA observed cracks on the oudoor surface area ,which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observations , records review and interview , the licensee did not comply with the section cited above, 3 out 5 records were missing Mandated Reporter Certifictes , which poses/posed a potential health, safety or personal rights risk to persons in care.
