Personal Rights — Based on interviews and S3's admission, the licensee did not comply with the section cited above in that it was determined on 6/18/26, S3 used inappropriate discipline and violated C1’s Personal Rights, which poses an immediate health and safety risk to clients in care.

101229(a)(1) Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary...No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. Based on interview and record review, the licensee did not comply with the section cited above in that it was determined that on 10/27/25 there was a Lack of Supervision resulting in C1 being unsupervised, which poses an immediate health and safety risk for persons in care.

Responsibility for Providing Care and Supervision — Based on LPA interviews conducted, the licensee did not comply with the above regulation, as incidents reviewed indicated multiple bite injuries with children as a result of a lack of supervision by staff, which poses an immediate Health & Safety risk to children in care.

Personal Rights — , Based on review of video footage, staff interviews, and facility record review, at the time of this incident facility staff did not ensure the safe and proper handling of children which resulted in injury to a child that required medical attention and posed an immediate Health and Safety Risk to children in care.

Buildings and Grounds — Based on interviews conducted and record review the licensee did not comply with the section cited above as the facility had damaged window screens and evidence of a prior water leak that resulted in a musty odor which poses a potential health, safety or personal rights risk to the children in care.

Teacher-Child Ratio — Based on interviews and record review the facility did not comply with the section above as on May 12, 2026 transition logs reflected that there were 25 children supervised by one (1) staff member for approximately 25 minutes, resulting in the classroom operating out of ratio during that time.

Observation of the Child — Based on LPA interviews conducted, the licensee did not comply with the above regulation, as staff neglected to notify C1’s parents of at least one (1) injury incident in a timely manner which poses a potential Health & Safety risk to children in care.

Outdoor Activity Space — , Based on observation, interview, and record review, facility did not ensure a safe enviroment for the activities planned by allowing children to ride tricycles on the preschool playground near a surface that sloped downward towards a metel shade canopy, which posed a potential health and safety risk to children in care riding tricycles.
