Teacher Qualifications and Duties — Based on observation, interview, and record review, the licensee did not comply with the section cited above by having staff Ashley Buckley supervising 12 preschool children alone while not being a fully qualified teacher which poses an immediate health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — LPAs observing children using the restroom alone on two different inspection days with a lack of supervision. This is an immediate health and safety risk to children in care.
Teacher-Child Ratio — LPA Vang leaned from the staff interviews that the facility has been operating out of ratio in more than one occasion. LPA Vang also witnessed S1 supervising 16 children in the yard for 6 minutes. This is an immediate health and safety risk to children in care.
Personal Rights — Based on observation, interview, and record review, the facility did not comply with the section cited above by having an incident where Staff #1 handled a child in a rough manner during naptime which poses an immediate health, safety or personal rights risk to persons in care.
Teacher-Child Ratio — LPAs observed 2 classrooms operating out of ratio. Classroom 1 had a census of 14 children and 1 staff, Classroom 2 had a census of 15 children and 1 staff, which poses an immediate health, safety, or personal rights risk to persons in care.
Personal Rights — Director did not comply with the section cited above as interviews, witness statements, and record reviews reveal that staff speak inappropriately to children and staff handle children roughly, which poses an immediate Health, Safety, or Personal Rights risk to persons in care.

Buildings and Grounds — Through multiple inspections, LPAs observed left over food in a bucket accessable to children, and diaper bins with no trash lids. LPA observed feces on the toilet seat on 11/4/2025 and residues on a step stool and floors of the Two's classroom. This is an immediate health and safety risk to children in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above by not having a valid Mandated Reporter Certificate for all staff in facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above by having 40% of all staff files reviewed missing an LIC503 Health Screening report from a physician which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — LPAs observed pests in facility during inspections on 28 August 24 and 18 September 24 which poses a potential health, safety, or personal rights risk to persons in care.
