At 9:11AM LPA observed 19 children being supervsied by S#2, S#13 in the"twos room". Both instances the facility was found to be out of ratio. — requirement not met

Besed on LPA document review and interviews conducted, this requirement was not met, on 10/18/2024 C#1 was left without care and supervision for less than two minutes. And on 4/20/2023 Child#2 was left without care and supervsion for about 2-3 minutes.

S1 had a incomplete file/personal record. S1 wa missing the aforementioned documents.
