The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. Based on observations and interviews collected the facility has been found to be out of compliance due to the following .. At 8:59AM LPA observed S#13,S#17 supervising 18 children in the "twos room". At 9:11AM LPA observed 19 children being supervsied by S#2, S#13 in the"twos room". Both instances the facility was found to be out of ratio. — requirement not met

Health-Related Services — Based on observation, and record review, the facility did not comply with the deficiency cited above, in having incomplete first aide kits which poses a potential risk to the health, safety and or personal rights of children in care.
Teacher Aide Qualifications and Duties — Based on interviews conducted, and record review, the facility did not comply with the deficiency cited above in having the food specialist supervise napping children, which poses a potential risk to the health, safety and or personal rights of children in care.
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, ... Supervision shall include visual observation. Besed on LPA document review and interviews conducted, this requirement was not met, on 10/18/2024 C#1 was left without care and supervision for less than two minutes. And on 4/20/2023 Child#2 was left without care and supervsion for about 2-3 minutes.
The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (3) Date of employment.Documentation of the educational background, training and/or experience specified in this chapter. (11) A health screening as specified in Section 101216(g)..... Due to file review, LPA's observations, this regulation was not met on 12/12/2024 by the Licensee. S1 had a incomplete file/personal record. S1 wa missing the aforementioned documents.
