Limitations on Capacity and Ambulatory Status — Based on record review and staff interviews, the facility was using a room that was not licensed, and they have 16 children enrolled in their classroom. This poses a potential health and safety risk to the children in care.
Materials provided to an applicant at orientation — Based on LPAs observations and staff interviews, the facility moved children to classroom Prek2 due to LPA being present and the principal stated that the principal covers breaks for staff members which does not occur. This poses a potential health and safety risk to the children in care.
Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above, Director confirmed facility has not completed lead testing for their water outlets, which posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff memebrs, both Staff #1 and Staff #2 did not have current Mandated Reporter training certificates, which poses a potential health, safety or personal rights risk to persons in care.
