Child Care Center Director Qualifications and Duties — Based on interviews and record reviews the facility did not comply with the section cited above when LPA conducted a visit on 11/13/2025 and .... LPA was told that S1 was an "Acting Director" until the position was filled, which S1 later informed LPA that they were just a Teacher and the facility currently did not have a Director. During this period multiple incidents have occured which poses an immediate risk to the health, safety or personal rights of children in care.

Criminal background check — Based on observation, interview, and record review, the licensee did not comply with the section cited above by not ensuring all staff have an eligible criminal background clearance prior to working in the facility, which poses/posed a potential health, safety or personal rights risk to persons in care. Bouchra Chahboune did not have an eligible criminal background clearance during today's visit. The Administrator was informed to check Guardian or contact the Oakland office to very clearance.

Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above by not ensuring all staff have the required immunization records in their file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — LPA observed the children in the classroom alone, while staff was in another classroom on two separate occasions, which is a potential risk to the health, safety, and personal risk to the children in care. Both times, the staff immediately returned to the classroom.
