LPA observed 2 changing pads with plastics on them that were worn out, 1 had holes at the ends and the other had tape on it. This poses a potential health, safety or personal rights risk to persons in care.
Notice to parents that site visit reports are available — Based on children records reviewed and interview, Child #2 was missing a LIC 9224 acknowledgement form receipt dated 3/13/2024 with a Type A citation. Form was not provided to Parents. This is a potential safety risk to the children in care.

During outdoor physical plant inspection, LPA observed the bottom part of a section of the fence surronding the infant play yard rustic, rotten, and with holes. This poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above. LPA observed there were no completed individual Infant Safe Sleep Plan (LIC 9227) form in their files for 5 out of 5 infants. This poses/posed a potential health, safety or personal rights risk to persons in care.
Modifications to Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above. LPA observed 3 out of 5 infants did not have an updated needs and services plan in their file. This poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above. LPA observed Child #3 did not have an updated immunization record on file. LPA observed child #3 did not have the measles vaccine on record. Child #5 did not have a complete immunization record on file. This poses a potential health, safety or personal rights risk to persons in care.
