Responsibility for Providing Care and Supervision for Infants — staff admitting and documentation observed noting staff had been sleep during the time that visual supervision was to be conducted.

Personal Rights — (S1) disclosed during the interview I grabbed (C1) by the arm and bit him. I left a bite mark on his forearm. I don’t recall which arm. (S2) saw what I did, and she did not condone what I did. This is something that I never done in my life being a teacher.

Responsibility for Providing Care and Supervision — Based on staff interview statements, staff did provide visual observation which led C1 and C2 to demonstrate serious inappropriate behavior. This is an immediate risk to the health and safety of children in care.

Personal Rights — Interview statements from staff disclosing that a staff member was heard yelling a child in the Director's office. If not corrected, this poses a potential health and safety risk to children in care.

Personal Rights — During staff interviews, S3 admitted she accidentally close child's finger in the door hinge, which poses an immediate health, safety or personal rights risk to persons in care.

Activities/Napping — During staff interviews, S1 stated C1 was having a hard time napping and kept moving away from his mat. S1 admitted to picking child up and physically placing him on his mat, which poses a potential health and safety risk to children in care.

Reporting Requirements — An unusual incident occurred at the facility on 2/19/2025 and parent reported to the Director on 2/20/2025. The facility reported incident to CCLD on 2/24/2025. This is a potential risk to the health and safety of children in care.

Personal Rights — S1 and S3 disclosed that a child was hit by a half door in the classroom. Staff have witness other staff arguing in the presence of children and staff do not monitor child's illness. If not corrected, this poses a potetnial health and safety risk to children in care.

Infant Care Teacher Qualifications and Duties — S1 did not have infant and toddler units and was allowed to care for infants, act as a lead teacher and supervise 2 other staff. If not corrected, this poses a potential health and safety risk to children in care.

Reporting Requirements — An unusual incident occurred at the facility on 9/5/2024 at approximately 10:00am. The Director at that time reported the incident to CCLD on 9/6/2024 at approximately 2:00pm. This is a potential risk to the health and safety of children in care.
