During the staff file review, the LPA was unable to review a current Mandated Reporter certificate for S1, S2, or S3. S1 and S2 had expired certificates. S3 did not have one on file.

During the staff file review, the LPA was unable to review a current EMSA approved CPR/1stAid certificate for S1, S2, or S3.
Staff immunization — influenza, pertussis and measles — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the staff file review the LPA was unable to review immunizations (DTap/MMR) for S3.
Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review, the LPA was unable to review an LIC 9227 Individual Infant Sleeping Plan for C1.
