Infant Care Activities — Based on LPA's observation an infant was in a crib, asleep on their stomach which poses an immediate health, safety or personal rights risk to persons in care.

(1) Placement of cribs, mats or cots shall not hinder entrance or exit to and from the napping space. This was not met as evidenced by: Based on LPA interview and record review, A2 was instructed to place the crib in front the baby gate by the door in the nap room and stay in room 2 with the infants who were awake. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care.
(d) The surface of the outdoor activity space shall be maintained: (1) In a safe condition for the activities planned. This was not met as evidenced by: Based on LPAs interviews and observations, A4-A7 observed children putting rocks and/or bark in their mouths. Addionally, LPAs observed one infant putting dried chickpeas in their mouth. The facility did not ensure outdoor activity space was in a safe condition for the activities planned which poses/posed a potential health, safety and/or personal rights risk(s) to children in care.
The infant shall be fed in accordance with the individual plan. This is not met as evidenced by: Based on LPAs record review and interviews with A4,A6, C1’s bottle was served to another child. This poses/posed a potential health, safety and/or personal rights risk(s) to children in care.
