Infant Care General Sanitation — Based on LPA observation and inspection the infant toys located outside in the designated infant outdoor play area are not clean and have sticky residue, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Infant Care Center Sleeping Equipment — Based on LPA Lovest observed S1 place C6 on the crib where a portable sound machine was attached to the side of the crib, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care General Sanitation — Based on observation the facility did not comply with the section cited above,LPAs observed 4 infants diapers were changed by S5 and 1 infants' diaper changed by S3. S5 and S3 did not wash hands after changing infants' diapers. LPAs observed 4 infants fed by S1 and 2 infants fed by S8. Based on observation of S1 and S8, did not wash their hands before the feeding which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on observation the facility did not comply with the section cited above,10 bottles of milk did not have a date labeled on bottle, 2 of 10 bottles of milk observed did not have a name or a date labeled on milk bottles, 10 of 10 Infant personal Food containers were labeled with childs name and no date during todays visit which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation the facility did not comply with the section cited above, 4 of 4 infants observed being changed on a infant changing table that is covered by a padeded non vinyl or plastic surface which poses/posed a potential health, safety or personal rights risk to persons in care.
