Buildings and Grounds — Based on observation the licensee did not comply with the section cited above in where during the tour of the facility LPA observed Rm 2, Rm 4 changing tables with dirt/grime, floor boards and walls with dirt/grime Rm 1-4, 5 outlet covers missing between Rm 1-4, which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation the licensee did not comply with the section cited above in cleaning solutions, exterior building paint, bleach/water mix, room sprays and disinfectants were accessible to children in rooms 1-4 as the areas kept did not have child safety locks and/or within reach of children, which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Aide Qualifications and Duties — , Based on LPA's observations and interviews conducted Staff #1 (S1) stated they were giving a teacher a break on 7/16/25 and was alone with 12 children even though they have no units. This is a potential health and safety risk to the children in care.
Limitations on Capacity and Ambulatory Status — , Based on interview conducted and LPAs observation, it was disclosed that C1 was moved from the infant classroom into the 2's classroom prior to their 2nd birthday to meet ratio. This is a potential health and safety risk to children in care.
Buildings and Grounds — , Based on LPA observation in the kitchen, LPA observed mice droppings and 1 dead cockroach in the pantry area. Lastly, based on record review of pest control receipts, it was documented that pest control had seen pest activity over the past few months and was asked to treat for mice and cockroaches based on activity being reported. This is a potential risk to the health and safety of children in care.
Food Service — , Based on interviews, 4 out of 4 staff memers stated C1 was served food with known allergens. This is a potential health and safety risk to the children in care.
Carbon monoxide detectors required — Based on observation, there was no working carbon monoxide located, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Indoor Activity Space — Based on observation, the flooring in hallway was in disrepair with tears, missing sections and cracks, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation, furniture, bathroom door, bathroom toilet paper dispensers, and drywall are in disrepair, therefore licensee did not comply with the section cited above in 4 items, which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, plumbing fixtures for the preschool room are not in proper working order, therefore the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observations and 2 out of 3 interviews, outdoor activity space ground surface is not maintained and poses a potential health and safety risk to children in care.
