Infant Care Food Service — Based on interview and record review, on one date a child was not offered their second bottle feeding. The Individual Feeding Plan stated the child would be offered a bottle 2 times during the day at set times. This poses/posed a potential health, safety, or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interview & record review, the licensee did not comply with the section cited above in that on two different dates a child sustained a minor injury and no staff member witnessed the injury take place. This poses/posed an potential health, safety, or personal rights risk to persons in care.
Infant Care Food Service — Based on interview and record review, on one date a child was not offered their final bottle feeding of the day. Additionally, the Individual Feeding Plan stated the child would be fed every 3 hours, and it had been 3 hours and 30 minutes since the last offered bottle. This poses/posed a potential health, safety, or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on observation and interview, the licensee did not comply with the section cited above in that in the Toddlers A and Toddlers B room there was no separation between the indoor activity space and the sleeping space. Additionally, there is no waiver or waiver request to waive this requirement. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that 1 out of 5 staff files (Staff 4 [S4]) contained a Mandated Reporter Training certificate that expired 3/2026 and a renewal Mandated Reporter Certificate is required. This poses/posed a potential health, safety or personal rights risk to persons in care.
