Criminal background check — Based on interview and record review, four employees prior to employment or initial presence in the CCC were not properly associated or granted fingerprint clearances. Where S4 worked six (6) days and was not cleared. S5, S6 and S7, fingerprint applications were initiated but not cleared prior to presence in CCC. Which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — , based on record review and interview, Child #1 was left in their crib unattended in classroom Infant B for approximately 7 minutes while teacher was walking children in the buggy and another parent found child waking up.This poses an immediate risk to the health and safety of the children

Limitations on Capacity and Ambulatory Status — , Based on interviews conducted 2 of 2 staff memebrs disclosed that they would combine the infants and preschool aged children at the end of the day in the infant room. This is a potential health and safety risk to the children in care.

Admission Policies — , Based on interviews conducted 3 of 3 staff members in the classroom on 5/5/25 stated a red mark was seen but no incident report was written nor were the parents informed. This is a potential health and safety risk to children in care.

Personal Rights — , Based on interviews conducted staff were aware item on treehouse was broken for some time, however, actions had not been taken to fully repair it. This is a potential risk to the health and safety of children in care.

Staff-Infant Ratio — , Based on interviews conducted, staffs understanding is if children are awake but on their mats they can have a 1:4 ratio. Staff have confirmed that this has been the practice during nap time. This is a potential risk to the health and safety of children in care.

Infant Care Food Service — , Based on interviews, it was disclosed that staff would feed infants personal food that was not served from the facility or a part of the food program. This is a potential risk to the health and safety of children in care.

Personal Rights — , Based on interviews, it was disclosed that staff speak inappropriately to infants by calling children names other than their own. This is a potential risk to the health and safety of children in care.

Personal Rights — , based on interviews it was disclosed that staff handle infants in a rough manner by moving the infants in a fast manner from one spot to the next. This is a potential risk to the health and safety of children in care.
