Health-Related Services — Based on information obtained during interviews and records reviewed, it was revealed that staff did not seek timely medical attention for C1’s serious head injury which poses an immediate health risk to children in care.
Criminal Record Clearance — Based on observations, the facility did not comply with the section cited above as a staff member was working without a criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interview, the facility did not comply with the section cited above as a child sustained an injury in the classroom that required medical treatment. Additionally, the incident was not observed by staff members which poses an immediate health, safety or personal rights risk to persons in care.

This regulation was not met as evidenced by: Based on evidence gathered through interview & record review, the licensee was unable to provide proof of required notifications to authorized representatives of Type A violations issued on 04/22/2024, 05/02/2025, 12/29/2025 and 03/05/2026, which is a potential risk to health/safety of persons in care.
101215.1 In part...(f) When the child care center director is absent from the center, arrangements shall be made for a fully qualified teacher as specified in Section 101216.1(c) to act as substitute. This regulation was not met as evidenced by: Based on evidence gathered, through observation, interviews, and records, the licensee designated unqualified staff to facilitate daily operation in the absence of the director or licensee, which is a potential risk the health and safety of children in care.
101229 (a) The licensee shall provide care & supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...(1). Supervision shall include visual observation. This regulation was not met as evidenced by: Based on LPA observations of lack of supervision at 10:10 AM, when 2 children used the restroom without supervision, interview & by the licensee's admission that daycare children are allowed to use the restroom independently, which is a potential risk to the health & safety of children in care.
(1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure.
