At approximatly 12:37pm LPA Castellanos observed 23 children, 13 of whom where awake, being supervised by one staff member, which poses an immediate health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on observation, interviews, and record review, LPAs observed staff 4 interacting with children. The director did not ensure a criminal record clearance was obtained for Staff 4 (S4), which poses an immediate health, safety, and personal rights risk to children in care.

Buildings and Grounds — Based on observation, the facility did not comply with the section cited above, LPA observations done on 6/26/2024 it was found that the facility staff do not keep classrooms clean and sanitary, which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on observation, interview, and record review, the Director did not ensure proper sign in for 24 out of 74 children which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interview statements and record review. Staff acknowledge miscommunication between staff and parents regarding C1 diapering and toileting needs which resulted in C1 being left in a soiled pull-up throughout the day which poses a potential health, safety, or personal rights to persons in care.
