Personal Rights — , Based on information obtained in staff interviews and LPA review of video footage of the incident, facility staff did not meet this requirement when Child 1 (C1) was handled roughly by Staff 1 (S1), which posed an immediate risk to the health, safety and personal rights of children in care.

Responsibility for Providing Care and Supervision — , based on interviews and video footage review, the facility did not comply with the section cited above, when C1 and C2 were left unsupervised on the Infant Playground for 44 seconds resulting in C1 biting C2 six times, which posed an immediate Health, Safety and/or Personal Rights risk for children in care.

Infant Care Food Service — Based on observation and interview, the licensee did not comply with the section cited above in 2 out of 5 persons as the bottle label for Child 1 and Child 5 did not have the date which poses a potential health, safety or personal rights risk to persons in care.
