Responsibility for Providing Care and Supervision — ; On 03/27/2025 a child sustained injuries requiring medical treatment, by staffs own admission, corroborated interviews and video evidence, staff failed to provide supervision at the time of the injury.

Fire Clearance — ; Based on observation and interview, the licensee did not comply with the section cited above where the licensee did not ensure that the fire protection system was not in working order at all times, which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews, it was revealed a child had wandered away, unnoticed, and without supervision for a period of time between 1-5 minutes. This poses an immediate health and safety risk to the children in care.

Responsibility for Providing Care and Supervision — Based on interviews with 3/3 staff, interviews revealed child was left out on playground for at least 5 minutes. This poses an immediate health and safety risk to the children in care.

Responsibility for Providing Care and Supervision — ; Based on evidence received, interviews conducted and video footage reviewed, on May 26, 2026 a child sustained a fall from the outdoor play structure whilst a second child placed bubble toy with bubble solutions in their mouth, at which time staff were not visually observing and supervising either of the children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews and evidence obtained, the Licensee did not comply with the section cited above in ensuring that the children in care are accorded dignity in his/her personal relationships with staff and other persons when a staff was observed to have physically restrained a child in care, which posed/poses a potential health, safety and/or personal rights risk to children in care.

Food Service — ; Based on observation and interview, the Licensee did not comply with the section cited above it was observed that the food served to children in care was stored improperly and prepared in the school age classroom cubby area, including use of the school age handwashing sink, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on interviews, evidence collected and records reviewed, the licensee did not comply with the section cited above where it was found that Staff did not ensure the center was free of pests, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — , Based on interviews conducted and video footage observed, S1 and S2 both were observed to mishandle C1 causing C1 to cry and did not offer support. This is a potential health and safety risk to children in care.

Personal Rights — , Based on interviews conducted and video footage observed, S1 and S2 both were observed to mishandle C1 by grabbing and pushing C1 causing C1 to fall to the ground. This is a potential health and safety risk to children in care.

Responsibility for Providing Care and Supervision — ; On 12/17/25 a child sustained unexplained injuries to the face while in care of the CCC, which staff interviews revealed to be due to lack of supervision which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — ; Based on interview and record review, the licensee did not comply with the section cited above when an outbreak of communicable diseases were not reported to CCL, which poses a potential health, safety or personal rights risk to persons in care.

101219(f) Admission Agreements (f) The licensee shall comply with all terms and conditions set forth in the admission agreement. The requirement was not met as evidenced by; Based on observation, interview and record review, the licensee did not comply with the section cited above where the CCC did not follow the terms set forth in their agreements on pages 26 and 27, specifically, which poses a potential health, safety or personal rights risk to persons in care.

101238 (a) Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The requirement was not met as evidenced by; Based on observation, interview and record review, the licensee did not comply with the section cited above where it was revealed that staff are not following the cleaning processes by which the CCC stated are used, were not made aware of what those specific processes are nor given sufficient time to clean or cleaning supplies to use, which poses a potential health, safety or personal rights risk to persons in care.

Admission Agreements — ; Based on interviews, evidence collected and records reviewed, the licensee did not comply with the section cited above where it was found that the allegation Facility does not follow admission agreement policies is Substantiated, Which poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation, interview and record review the licensee did not comply with the section cited above in 1/1 children requiring an epipen did not have the medication maintained and on site, which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — ; Based on record review, the licensee did not comply with the section cited above in Licensee did not obtain signatures on LIC9224 from Type A citation from annual visits 02/2025 and 04/2025 for all enrolled children which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview the licensee did not comply with the section cited above, where LPA observed an unlatched approx. 2.5 ft tall gate opening to 2 sheds, with open doors, one shed contained children’s outdoor materials and items used daily and shed two with open doors contained 4 hedge trimmers, several bottles of weed killer, bleach, multiple cans of house/building paint and other solvents/agents labeled toxic. Aside from doors fully open/ajar, shed also did not have a lock present, which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — ; Based on observation and interview the licensee did not comply with the section cited above, LPA observed the outdoor activity space for preschool does not have any shade available to children. The CCC has a blue line across cement area that children are instructed not to cross, which is where the only shade available to children is located, under a building roof overhang outside of classroom 6, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — ; LPA observed 4 large gopher holes that were located in the outdoor activity space, with mounds of burrowed dirt over 6 inches, spilling over onto bike path and other activity areas for the children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — ; LPA observed that 3/3 preschool classrooms in use did not have a toilet paper dispenser, where the wall next to the toilet had evidence of the removed dispenser only. 2/3 classrooms had available toilet paper for children to use located on an upper window ledge, approximately 5 foot high and not accessible by children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in where classroom number 6 bathroom, which included the toilet and ceiling, were in disrepair until todays date. Additionally, the fire alarm is not in proper working order, classroom #3 back door does not lock allowing access to outside alley of the CCC, Classroom #5 and # 2 front door handle are not in proper working condition, which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, 3/3 preschool classrooms (Rooms 4, 5, 6) found to have chipping paint on indoor walls, cabinets chipped and in disrepair and childrens furnishings in disrepair.

Napping Equipment — During the tour of the facility, LPAs found that 8/10 nap mats used in classroom 4 were found to have ripped vinyl, sharp edges due to the rips in the vinyl and/or exposed foam in areas of the cracked and ripped vinyl

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in where 2/3 children’s toilet in the preschool facility were not in proper working condition and 3/3 classrooms in the preschool were found with to have visible dirt, grime and residue on the sinks, floors, walls and cabinets of the facility.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, the licensee did not comply with the section cited above in where the garden portion of the outdoor activity space contains a garden box with protruding nails, screws, rotted wood pieces, debris and a wooden bench in disrepair also with protruding nails and broken wood pieces which poses/posed a potential health, safety or personal rights risk to persons in care.
