Outdoor Activity Space — Based on observation of playground, the licensee did not comply with the section cited above in uneven cement walkways which poses a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 child records reviewed were missing a physican's report which poses a potential health, safety or personal rights risk to persons in care.
