10149.2(b)(2) Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20)], which is incorporated by reference. — Based om interviews, observation, and record review, the facility did not comply with the section cited above in that, the LPAs observe C2 sleeping on their stomach and verified that the LIC 9227 section C was not filled out. Which poses/posed a potential health and safety or personal rights risk to children in care.

Responsibility for Providing Care and Supervision — … Based upon incident report received on 7/15/25 , video review, and interview with Director, children C1 and C2 were left unsupervised for approximately 30 seconds after exiting the classroom, which is a potential health, safety and personal rights risk to children in care.

Responsibility for Providing Care and Supervision — … Based upon unusual incident report submitted by the facility and interviews with staff, C1 was left unattended outside the classroom and Directors Office, for approximately 2 minutes, which is a potential health, safety and personal rights risk to children in care.

Fixtures, Furniture, Equipment and Supplies — Based on LPA's observation, the sink in Infant Room #2 was found to have molds around the faucet, creases and crevices area.

Personal Rights — Based on the fact that Child #1 was given Child #2's bottle to drink. This posed a potential risk to the health and safety of the child.
