Buildings and Grounds — Based upon LPA observation and facility unusual incident report - a brick pillar in the classroom was accessible and not covered causing injury to a child that fell and hit head on the ledge which is an immediate health, safety and personal rights risk to children in care.

Current roster of children — Based on record review, the licensee did not comply with the section cited above, as the facility did not maintain an updated facility roster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — … Based upon interview and observation, it was determined that facility did not maintain sufficient variety and quantity of toys, equipment, and materials, which is a potential health, safety and personal rights risk to children in care.

Child Care Center Director Qualifications and Duties — Based on interview and observation, the facility director did not obtain the required preventative health certification, which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, staff member S2 is missing vaccine records for Tdap, MMR and Flu, which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews the facility did not ensure supervision of child C1, as child unaccounted for during outdoor activities, which posed a potential Health, Safety or Personal Rights risks to persons in care.
