Buildings and Grounds — Based on interview, footage observed, furniture was rearranged to create a hallway and divide two classrooms, but furniture was obstructing the classroom’s entrance and pathway for children to roam and play which posed a potential health, safety or personal rights risk to children in care.
Personnel Requirements — Based on staff interviews and file reviews, former staff did not prevent Child #1 from harming other children in care. This posed a potential health, safety or personal rights risk to children in care.
