Child Care Center Director Qualifications and Duties — LPA conducted inspections on 7/1/24,8/2/24, and 8/22/24 and there was no director and staff informed LPA there is no director.
Personal Rights — During interviews with 8 staff, staff stated they witnessed or heard Staff 1 (S1) and Staff 5 (S5) engaging in a verbal altercation in the presence of day-care children which poses a potential health, safety, or personal rights to children in care.
Napping Equipment — During inspection on 7/1/2024 LPA toured classrooms and observed ripped napping mats in toddler classroom which poses a potential health, safety, or personal rights to persons in care.
Teacher-Child Ratio — Staff interviewed stated they have been left out of ratio themselves and have witnessed other staff being out of ratio which poses a potential health, safety, or personal rights to persons in care.
Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above. Administrator could not provide lead testing results during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interview, the licensee did not comply with the section cited above. Administrator was unable to provide proof of the emergency drill log which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. LPA observed staff #1 and staff #2 did not have a current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. Staff #1 did not have immunization records available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
