School-Age Child Care Center Director Qualifications and Duties — Based on observation, interview, record review, the licensee did not comply with the section cited above in the center did not inform Community Care Licensing (CCL) of the the director change within 10 days which poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, LPA observed one toilet inside the boy's restroom not working properly and Staff disclosed the staff restroom was clogged as of 4/13/2025, and it was not reported to Licensing which is a potential health, safety, or personal rights risk to the persons in care.
