101215.1 Child Care Center Directors Qualifications and Duties (a) In addition to Section 101215, the following shall apply: (b) All child care centers shall have a director. (c) The child care center director shall be responsible for the operation of the center, for compliance with regulations, and for communications with the Department; and has the authority to acknowledge receipt of deficiency notices and to correct deficiencies that constitute immediate threats to children's health and safety. Based upon staff interviews the center does not have a completed designated Director packet to submit to the Department since the center’s previous Director, Allison Smith, who no longer works at the center effective 04/12/26.

Responsibility for Providing Care and Supervision — , Based on LPAs observation S1 sent C1 to use the restroom on opposite side of building unsupervised. This is a potential health and safety risk to children in care.

101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department…. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (C) Any unusual incident.…… Based upon staff interviews the requirement was not met by the center by not submitting reports by telephone or fax by the next working day and by not submitting in writing within seven days following the occurrence of such events for the lice outbreak that occurred on 02/18/26 involving multiple children and the flood that occurred on 02/16/26 on the premises.
