Criminal background check — Based on interview and record review, the licensee did not comply with the section cited above as the facility did not report an usual incident where a child required medical treatment.

The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... Based on interviews, the licensee did not comply with the section cited above in P1 yelled at both C2 and C3 in the Tiger classroom which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation, 2 children were without visual supervision in the restroom, which poses a potential health and safety risk to children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 2 staff have only the Mandated reporter general training but not the complete child care provider training which poses/posed a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above in not having water tested for lead which poses/posed a potential health, safety or personal rights risk to persons in care.

Napping Equipment — Based on observation, the licensee did not comply with the section cited above in all nap equipment was stored where the bedding was in contact with others bedding which poses a potential health, safety or personal rights risk to persons in care.
