Buildings and Grounds — , based on observation. LPA observed a shed in the back play area which is sitting on blocks and has an opening on the bottom of the shed which children are able to crawl underneath, which is unsafe for children in care.

Responsibility for Providing Care and Supervision — Based on interview and record review, a staff turned around and bent over to cover the sandbox, and one of the children standing beside her scratched the other child standing beside her. The teacher did not observe the incident occur, which poses a potential health and safety risk to children in care.
