Personal Rights- The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidence of FCCH Assistants not being properly trained on what to do in case of the injury to C1 5/1/24.
Revocation or Suspension of a License or Registration — FCCH Assistants informing LPAs Licensee is always at the facility 5/10/24 and 5/15/24, which contradicts the fact Licensee was not at FCCH the required amount of time from 4/1/24 to 5/1/24.
Staffing Ratio and Capacity — During the inspection on 5/15/2024, LPAs observed 15 children in care. The review of 5/14/2024 attendance record revealed that FCCH had total of 16 children present 5/14/2024 This poses an immediate risk to children in care.
Large family home — conditions for caring for 13 or 14 children — During today's inpection and review of FCCH attendance record, LPAs observed 15 children present (reduced to 13) whose ages ranged from 3 to 5 years and there were no school age children among these children. This poses an immedite risk to health and safety of children in care.
