Health-Related Services — staffs' interviews that C1 fell backwards on the head. C1 was tired and sleepy during the day. Ice pack applied. Child’s representative was not notified. This is an immediate risk to the health and safety of children in care.
Criminal background check — Based on observation and record review, the licensee did not comply with the section cited above in 5 count out of 21 which poses an immediate health, safety or personal rights risk to persons in care. Staff Annabelle Miller, Vance Marini, Chloe Lesko, Marisol Ramirez, and Tatiana Tobin did not have fingerprinting documents on file with no clearance ID number. According to director, they are sent home to be lives canned.

Health-Related Services — interviews that the C1 fell on back of the head. Child was tired and sleepy tired and sleepy during the day. Child was sleeping on the bare floor of the basketball court and in the Member’s Activity Room on the hard wood floor for approximately 20 minutes on different timing. This is a potential risk to the health and safety of children in care.
Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above in one count out of one which poses/posed a potential health, safety or personal rights risk to persons in care. The childcare facility has not done the water lead testing yet.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 7 counts of 11 counts which poses/potential health, safety risk to persons in care. Staff # 3, 4, 5, 6, 8, 9. and 10 did not have a mandated reporter certificates on their file.
