Staff-Infant Ratio — Based on interviews with staff, the facility did not comply with the section cited above as the facility operated out of ratio which poses a potential health, safety or personal rights risk to persons in care.
Written directives for drinking water lead testing — Based on record review, the facility had 1 outlet in the Toddler Room (Orange) that had an ALE of 5.5ppb or greater which poses a potential health and safety risk to children in care.
Responsibility for Providing Care and Supervision — Staff were not positoned appropriately to ensure all children in care remained under visual supervision resulting in a child in care getting injured which is a potential health and safety risk to persons in care.
