Responsibility for Providing Care and Supervision — Based on interviews with staff and self reported unusual incident, it was determined that a 4 year old child was found outside of elementary school premises approximately 67 yrds passed the school gates, without visual supervison from staff. This occured at pick up time while the main street where the child was found, was busy with traffic. This poses an immediate risk to the health, safety and personal rights of child in care.

Outdoor Activity Space — Based on observation, facility did not comply with regulation in that there was a potted artichoke plant with small spikes accessible to children on the playground. This is a potential risk to the health and safety of children in care.
Reporting Requirements — Based on interview and documentation provided, facility failed to report an unusual incident to the department within 24 hrs via telephone. Incident ocurred on 2/05/25 and reported via phone on 2/07/25. This deficiency posed a potential risk to the health, safety and personal rights of children in care.
Notice to parents that site visit reports are available — Based on interviews, licensee did not ensure that each parent that signed LIC 9224 was provided copy of the licensing report dated 05/08/24, which poses a potential Health, Safety, or Personal Rights risk to children in care.
Personal Rights — … Based upon staff interviews, a verbal altercation occurred between staff members in front of parents and daycare children on 4/29/24 which is a potential health, safety and personal rights risk to children in care.
Teacher Aide Qualifications and Duties — … Based upon staff interviews, assistants were requested to do activities with groups of children outside the classroom without a qualified teacher numerous times which is a potential health, safety and personal rights risk to children in care.
