Family home — clean, safe, heated and ventilated — Based on observation, Upon at arrival during today's at 07:50am inspection LPA's smelled urine in the home. Smell was in playroom. While in the play room LPAs observed a puppy pad for dogs to use the bathroom. The Licensee cleaned it up. LPA observed Skat on the carpet where the pad was cleaned up. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, 4 out of 5 children's records reviewed did not have immunizations transcribed on CDPH 286. One child did not have immunizations up to date. The licensee did not comply with the section cited above which poses posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation,interview and record review, The roster shows 7 children enrolled. The 5 children that were present were not added on the roster. LPA gave Licensee the oppurtunity to add the children to have an accurate roster. Licensee reported it would take time away from provding services to children and would add them at a later time. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — admission procedures and parent rights — Based on observatio, interview, and record review, during record review 5 children's records were reviewed, and 4 children had incomplete files, with incomplete LIC 700, missing LIC 995A, missing LIC 627. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, interview, record review, the licensee was not documenting sleep logs for 23 month old infant in care. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, Licensee was not able to produce a valid copy of AB 1207 Mandated Reporter training certificate. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
