Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. At 10:00am LPA observed a small blanket with "lovey" stuffed animal head and a thin baby blanket in the play yard with infant C5 which posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, the licensee did not comply with the section cited above in 2 out of 2 staff files. Both S1 and L1 have expired Mandated Reporter training certificates and have not completed the Mandated Reporter training in the last two years, which poses a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 staff files. S1, and assistant who was caring for children without the Licensee present, did not have a current training certificate and had not completed the training within the last two years, which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interview, on 11/05/24 L1 was observed removing two different children from the hallway by picking the children up by their forearms to move them to another location, which posed a potential Health and Safety and Personal Rights risk to children in care.
