Teacher-Child Ratio — Based on LPA inspection, observing napping children, physical count of children, review of teacher assistant qualifications and teacher child ratio observation, LPA observed 15 children ages 18 months to 2 years old with one unqualified teacher assistant. THe licensee did not follow the regulation above which poses an immediate Health, Safety, or Personal Rights Risk to children in care.

Responsibility for Providing Care and Supervision — Based on LPA witness statements obtained through interviews.

Teacher Aide Qualifications and Duties — Based on LPA file reviews and observation at the center and witness statements, There are several staff members working at the center. without proof of qualifications for teachers and teachers assistants. Which poses an immediate Health, Safety, or Personal Rights Risk to children in care.

Health-Related Services — Based LPAs on observations and interview were Child #1 medication contain two syringes with liquid, that was labeled "Benadryl" with an expiration date 04/2024. Child #1 left the facility on 08/27/25, therefore this posed an immdiate health and safety risk to persons in care.

Health-Related Services — per director interview and record review Child 1, 2, and 3 no longer attend the facility. This posed an immdiate health and safety risk to persons in care.

Food Service — LPAs observations, at 10:55am LPA's observed a "brown-banded cockroach," in the corner of the kitchen. This poses an immediate health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — observations and directors statement. Per Director, parents use children's restroom. This poses an immediate health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on observations Licensee made structural changes to the licensed open concept classroom without notifying the department. Based on measurements taken during today's inspection the square fooptage for the preschool and toddler component has decreased. This posed an immdiate health and safety risk to persons in care.

Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Observation and interview.

Buildings and Grounds — Observation and interview.

Personal Rights — Based on LPA observations, observing child with severe cough during naptime, Interviews and witness statements, the staff is allowed to come in the center when ill and children are allowed to have contact with other children while ill, which poses/posed a potential Health, Safety, or Personal Rights Risk to children in care.

Mandated reporter training — Based on observation and record review, five staff need to renew the mandated reporter certificate, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation and record review, 12 staff were missing immunization records of various types, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on observation and record review, center uses the Bright Wheel for tracking naps, the app does not have the 15 minute checks during napping logged, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
