Infant Care General Sanitation — Based on observation and interview, the licensee did not comply with the section cited above. LPA and Director observed there aLysol disinfectant spral on the top shelf of a moveable cart, directly next to three infant bottles, that were without lids and had remaining milk, not preventing contamination. Which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space for Infants — Based on observation and interview, the licensee did not comply with the section cited above. Accessible to infant aged children on the playground, was a pretend kitchen, with an attached cunveyor belt that is not developmentally appropriate for infants in care. Futhermore, there was another toy kitchen that was broken and had an accessible sharpp broken edge. Which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, the licensee did not comply with the section cited above in nine of nine infant child sleep logs reviewed. When asked how sleep logs are documneted, it was disclosed that the position is not recorded. Which poses a potential health, safety or personal rights risk to persons in care.

Specifiaclly, Child 8 LIC 9227 was missing dates and signatures, disclosing if the child has the ability to roll over.
