Personal Rights — Based on staff interviews, LPA determined that a child exhibiting aggressive behavior had been “scooped” up by staff, an action described as holding the child behind the legs and back like a baby to carry them. This poses an immediate health, safety, or personal rights risk to persons in care.

Criminal background check — Based on observation, interview, and record review, the licensee did not comply with the section cited above due to a staff being present and having worked since 6/11/2024 withotut completing the personal background clearance process, and therefore did not have criminal background clearance. This poses poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — a self-reported unusual incident, confirmed through interview, in which a child spent 20 minutes alone and unsupervised in a fenced playground due to staff not following written protocol when transitioning children inside. This poses an immediate health, safey, or personal rights risk to persons in care.

Personal Rights — interviews and documentation that staff roughly redirected a child during nap time, grabbed a child by the shirt and made comments that can place fear to a preschool child which poses an immediate health and safety risk to the children in care.

Responsibility for Providing Care and Supervision — interviews that revealed two children were left outside unattended with no staff supervision and a staff member napping while supervising napping preschool children which poses an immediate health and safety risk to the children in care.

Teacher-Child Ratio — Based on review of video footage from the day 8/7/2023, LPA determined that a teacher was left alone with 13 children for several minutes. This poses and immediate health, safety, or personal rights to persons in care.

Teacher Aide Qualifications and Duties — Based on staff interviews and review of video surveillance on the day of 8/7/2023, the facility did not comply with the section cited above by allowing an aide who lacked formal educational or professional qualifications to work one-on-one with children outside of the classroom and beyond the supervision of a teacher. This poses an immediate health, safety, or personal rights risk to persons in care.

Personal Rights — Based on record review, the licensee did not comply with the section cited above in 3 out of 6 files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — No unusual incidents were reported to licensing regarding two children left with no supervision which posses an immediate risk to the children in care.
