Responsiblity for Providing Care and Supervision: a) The licensee shall provide care and supervision ...1)No child(ren) shall be left without the supervision of a teacher at any time...(1). Supervision shall include visual observation. Based on LPA interviews, staff did not provide care and supervision for two children who where left unattened at the facility playground. This is an immediate health and safety risk to the children in care.

Drinking water lead testing — Based on observation, record review, the licensee did not comply with the section cited above in that not all water dispensers at the facility received lead testing for lead leavels and still accessible to the child which posed a potential health, safety or personal rights risk to persons in care.
