101229 Responsibility for Providing Care and Supervision (1) No child(ren) shall be left without the supervision of a teacher at any time... This was not met as evidence by... Based on self-reported UIR, and interviews conducted, C1 followed a guardian out into the side yard. S1 only went out to get C1, when S1 observed a shadow through the window. This poses a potential health and safety risk to children in care.

Modifications to Infant Needs and Services Plan — Based on LPA's review of Infant Needs and Services Plans were revealed that several children's plans had not been updated quarterly. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
