Admission Policies — 101218.1 (e) At the time of acceptance of each child into care and for all children in car eon August 7, 2002, the licensee shall give each parent or authorized representative a copy of the Notificaiton of Parents' Rights(LIC995)...(1) The licensee shall request the child's parent or authorized representative to sign and date the acknowledgement -of-receipt statement at the bottom of the LIC995. The bottom portion of this form must be kept in the child's file as proof that the parent or authorized representative has been notified of his her her right by the child care center and received a copy. Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Three children's files were missing LIC995 Notification of Parent's Rights.
Teacher-Child Ratio — Based on observation, On October 29, 2024 LPA's did a physical plant inspection and observed Director stepped out of the outdoor playground leaving only one fully qualified techer (S2) and aid (S1) with twenty seven (27) children. LPA's witness the facility out of ratio again today one (1) aid was supervising nine (9) children in classroom #3. Based on records review staff (S1) is missing units to meet the teacher qualifications.
Mandated reporter training — Based on record review, the Director did not comply with the section cited above for staff (S1) & (S5) did not have a up to date Mandated Reporter Training Certification on file which poses a potential health, safety or personal rights risk to persons in care.
101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirment was not met as evidence by: LPA's conducted a facility tour and observed the urinal in the boys bathroom not in good repair with drain blockage.
