Outdoor Activity Space — Based on observation, interview and record review, the licensee did not comply with the section cited above in the outdoor flooring the in the infant playground was unsafe due to parts of the artificial grass was missing exposing the black material, rocks and dirt which poses a potential health, safety or personal rights risk to persons in care.
Staff-Infant Ratio — Based on observation and interview conducted, it has been determined that 8 infants were left in the room with 1 staff. This poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observations and interviews, four (4) staff stated the shade in the infant yard broke a few months ago leaving the area without shade This poses as a potential risk to health and safety of children in care.
General Sanitation — Based on observation, the rugs and mat in the outdoor area were damp and full of dirt and insects came crawling out of the rugs when inspected. There was a bucket with stagnant brownish water where the toys were.This poses as a potential risk to health and safety of children in care.
