Teacher Aide Qualifications and Duties — Based on LPA observations, record reviews, and interviews that were conducted the facility did not comply with the section cited above; from 8:20am to 9:05am, eight (8) children were present in Preschool Classroom No. 2 under the supervision of one aide (S3), which poses/posed an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on LPA observations, record reviews, and interviews that were conducted the facility did not comply with the section cited above and it was found that the facility was operating out of ratio, classroom 2 only had an aide an no teacher with 8 children in care, which poses/posed an immediate health, safety or personal rights risk to persons in care.

Admission Policies — Based on record review, interviews, and all evidence obtained, it has been determined that staff denied the authorized representative access to the facility on 12/03/2025 and did not provide access to the child’s records, which posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on interviews with relevant parties it was disclosed that the facility denied the authorized representative access to copies of C1’s record file, which posed a potential health, safety or personal rights risk to persons in care.

Teacher Aide Qualifications and Duties — Based on LPA observations, record reviews, and interviews that were conducted the facility did not comply with the section cited above; S3 does not meet the required qualifications to be alone with toddler-age children as observed by LPA throughout the day, which poses/posed a potential health, safety or personal rights risk to persons in care.
